FOHTA Written Consultation Questionnaire Form

This questionnaire gives the calligrapher an idea of what the client may want in terms of fonts, graphics, color, size, and estimated time of completion. By requesting a FOHTA Consultation Questionnaire Form the client is not obligated to purchase, but is entitled to a free estimate of his/her request.

FREE ESTIMATES will be given on addressing envelopes, fill-ins on certificates, signs, menus, original poems, prayers, quotes, letterhead design, business cards, and personalized greeting cards.

If the client agrees with the estimated price a deposit will be required to start production. Please fill out the form and the questionnaire will be sent to you via email or U.S. Mail.
